Welcome Letter

 

Dear Patient:

We would like to thank you for making an appointment with MidWest EyeCenter.

To relieve you from the necessary paperwork at the last minute in our office, we ask that you please complete the patient forms listed on our website. If you are pursuing cataract or refractive lens exchange, please be sure to complete that questionnaire as well. This will provide necessary information to our doctors and staff regarding your needs, desires and expectations. Please bring all completed forms with you on your visit.

Please plan on being at our office for approximately one and a half to two hours. Your eyes will be dilated; therefore, you may wish to arrange for someone else to drive you. You might also want to bring a pair of sunglasses to help with any excess light sensitivity from the dilation.

Your exam will consist of a complete eye examination, including a glaucoma check, a thorough medical evaluation for any other eye disease and a diagnostic refraction. The fee for the complete examination is $150.00 for new patients and $115.00 for established patients, which is due at the time of service. This fee includes a $20.00 refraction fee which is not covered by Medicare and may not be covered by other insurance plans. For those patients who have insurance we will file charges with your insurance company.

For those patients who are pursuing contact lenses, our office does not dispense or fit contact lenses. We can give you a prescription for glasses. To obtain contacts, you would need to present this glasses prescription to your optician or optometrist who will then fit you for contacts. Their office will likely charge you a fitting fee for this service.

For those patients pursuing refractive surgery, please keep soft contact lenses out for two weeks. Gas permeable, extended wear and hard lenses should be kept out a minimum of three weeks.

Because your old glasses could contain helpful information about your eyes, please bring any glasses you might be using to your examination. Also, please bring a complete list of any medications you are currently taking.
